Therapy Session: $150 / 60 minute session

HOW DO I GET REIMBURSEMENT FROM MY INSURANCE?

Crumbs and Smiles does not bill insurance companies. However, you can be provided with a super bill/invoice to seek reimbursement from your insurance company.

Contact your insurance and ask the following questions:

  1. Does my insurance reimburse out of network therapy services?
    • The primary CPT codes we use are: (insurance knows these codes) 92610 – Clinical Bedside Feeding and Swallowing evaluation & 92526 – Feeding and Swallowing Therapy
  2. If reimbursement is allowed, what is the process for me to submit my invoices to claims?
  3. Do I need to obtain a prescription from my child’s primary doctor for therapy services and include it with my claims?
  4. Which conditions, if any, are specifically excluded from speech treatment?
  5. Is there a limit on visits?
  6. If we are unable to get into an in network provider, or one is not available in our area, do you provide a gap exception so that we can be reimbursed at their network rate?
